All headteachers/proprietors of private secondary schools implementing USE (PPP Schools) are requested to submit data listed below by Monday 30th August 2010.

1. Copies of Registration Certifi cates and License. Also come with original copies.

2. Staff lists with the following details:
a) Name of the teacher
b) Date of birth
c) Registration Number
d) UTS Number
e) Qualifi cation e.g B.A/Ed
f) Subjects
g) Terms of employment
h) Teaching load
i) Responsibility in the school
v) Monthly Salary

3. A copy of the list of the Board of Governors approved by the
Minister.

Please note that failure to submit this information to the Commissioner, Private Schools, at UDC Building Floor 1 Rm. 20
by Monday 30th August 2010 will compel us to suspend the release of USE capitation grants to your school.
